The tournament will be played on the weekend of July 23-24 at Birmingham City University's Moor Lane Sports Ground, Moor Lane, Sutton Coldfield, Birmingham B6 7AA.
Teams that wish to enter should return the Application Form as soon as possible by post or email, following instructions on the form.
